What happens next?
Dr. Lim will personally review the information you submit to make sure your child’s needs are within the scope of the practice. If so, she will contact you for a brief phone conversation to answer any questions and help you schedule the appropriate visit, if desired.
Once scheduled, you will receive two separate portal invitations:
Charm EHR is our electronic health record. You will use Charm to provide basic demographic and payment information, complete your intake questionnaire and consent forms, and receive your Good Faith Estimate.
CHADIS is used for screening and diagnostic questionnaires based on your child’s needs. You can also use CHADIS to invite teachers to complete questionnaires when needed.
Please complete all requested forms at least 1 week before your appointment.
