This call is not a medical consultation and does not establish a physician-patient relationship.

What happens next?
Dr. Lim will personally review the information you submit and then contact you for a complimentary 15-minute phone call. She will answer questions about the practice, discuss whether your child’s needs are within the scope of services offered, and help you schedule a visit if appropriate and desired.

Once a visit date is selected, you will receive invitations to two separate portals:
Charm EHR is our electronic health record. You will use Charm to provide basic demographic and payment information, complete your intake questionnaire and consent forms, and receive your Good Faith Estimate.
CHADIS is used for screening and diagnostic questionnaires based on your child’s needs. You can also use CHADIS to invite teachers to complete questionnaires when needed.
A credit card on file is required to confirm your appointment. Appointments that are not confirmed at least 14 days in advance may be released to another family.
Please complete all requested forms at least 1 week before your visit.
